i had a simular incident but with a tee barrel sealer, i made a reducer out of some thick plastic to connect my 40mm pvc barrel to a 3/4 inch nipple, the golf ball got stuck in and blow the barrel off luckily it hit my sand box.

One of my first guns was a pneumatic gun with a "pump" bottle as a chamber and i putt a shotgun wad down the barrel at first fire it didn't fire it out.
The second time i fired it, it did the same so a friend and i opened up the valve and injected air straight into the chamber and barrel the chamber blow up at 95 PSI and i had a shradder valve attached to the end of the pump bottle, when it blew the force broke my wrist and the shradder valve impaled into my wrist just missing my main vain. The pressure put me in a cast and it also made my friend's ears feel like they were on fire.

I connected the hose from my fire extinguisher to the 3 way valve on my cannon, to see if it would pilot it to get semi auto, i didn't tighten the the compression fitting much and when i dumped pressure into the hose it came of with 350psi out the end and wiped me on the neck
it didn't hurt to much but left two nice cut

when i came back from the mirror, i looked at the hose and could see a bit of skin on the tip
